Applying for a card

Who is eligible?
To be eligible for a Tasmanian Companion Card, you must be able to demonstrate all of the following:
1. You live in Tasmania; and
2. You have a disability; and
3. Due to the impact of your disability, you are unable to participate at most community venues or activities without attendant care type support; and
4. Your need for this level of support will be life-long.

Who is a companion?
A companion may be a paid or unpaid person whose primary purpose for attending a venue or event is to provide attendant care support to enable the cardholder to participate in an activity or event. Cardholders can choose their companion, and this may vary depending on the activity and the occasion.

A person is not considered a companion for the purposes of the Companion Card program if they are required for only reassurance, social company or encouragement.

Application Process
Follow these steps to apply for a Companion Card:

  • Read the Tasmanian Companion Card Cardholder Handbook, which is available in other languages and accessible formats. This handbook will help you decide if you should apply. 
    If you choose to apply for a Companion Card, you must complete a Companion Card Cardholder Application Form. 
  • Attach two, high-quality, passport-sized photographs of yourself to your application form.
  • Have both your application form and your photographs signed by the relevant Health Professional / Service Provider as outlined in the application form.
  • Mail your form to the Disability Bureau, which will process your information and inform you of the outcome of your application. The people involved in your application may be contacted to verify the accuracy of the information provided.
