Replacement cards

You can request a replacement card if:

  • you lose your Companion Card;
  • your Companion Card is stolen or damaged;
  • you change your name;
  • your photograph needs to be up-dated.

To request a replacement card, submit a Replacement /Change of Details form.
Call the Disability Bureau Enquiry Line on 1800 009 501 to request a form.
